BioHealth SIBO Glucose/Lactulose Test: $299
BioHealth SIBO (Lactulose Only) Test: $199
Description:
The SIBO breath test is used in the diagnosis of overgrowth of bacteria in
the small intestine. Most of the bacteria of the gastrointestinal tract should be present in the large intestine, and while a small amount of bacteria should be present in the small intestine to aid in digestion and absorption, certain factors can lead to too much bacteria in the small intestine. This can lead to symptoms such as gas, bloating, and numerous other symptoms.
The SIBO breath test measures hydrogen and methane produced by the bacteria. Elevated hydrogen levels are commonly seen with faster transit times and IBS-D (diarrhea), whereas higher methane levels are usually associated with slower transit times and IBS-C (constipation). However, this doesn’t mean that everyone with high levels of methane will experience constipation and everyone with high levels of hydrogen will experience diarrhea.
Many labs only test for lactulose, and while this is an option, if you are testing SIBO for the very first time and/or haven’t tested for glucose in the past, testing for both lactulose and glucose might be a good idea. Both lactulose and glucose are common substrates used in a SIBO breath test, and the truth is that false negatives can happen when using either of these substrates alone, but having a false negative is greatly reduced when using them combined. Lactulose is usually preferred because it goes the entire length of the small intestine, which isn’t the case with glucose, as it may not be able to diagnose SIBO of the distal small intestine (ileum). However, different bacteria feed off of different substrates, and while every bacteria will feed off of glucose, not every bacteria will feed off of lactulose.
In my experience the lactulose part of the breath test is more likely to come back positive than the glucose, but I’ve also seen patients who test negative for lactulose but test positive with glucose. As a result, if someone relies on lactulose alone there is a greater chance of a false negative.

This test also looks at the circadian rhythm of cortisol by measuring the free cortisol and cortisone levels throughout the day. In addition, it tests for the cortisol metabolites, along with DHEA-S.
